Health Card Renewal Letter

Dear Employees,

We will be renewing our health card this April. We would want to get a consensus who among you intends to enroll your dependent/s. For your guidance in HMO industry, when we say dependents, it should be:

    If married - your husband/wife then kids
    If single parent - your kids then parents not above 65 years old
    If single - your parents not above 65 years old

Please let me know if you intend to enroll any of your dependents and specify if it's your kids (kindly indicate how many) / husband or wife /parents.

Restaurant Notice: Passed Health Department Inspection Letter
